I presume you're asking about an insurance claim.
Ask for a receipt after your massage session. If you don't get one, it may mean the place you went doen't have the kind of licence you need for an insurance claim. If you do get your receipt, submit it to your insurance company as you would any other insurance claim. If you don't know how, contact your HR department.

It's a licence for a body rub parlour. It means you don't have to worry too much about getting busted.

The only massage parlour I know of that is accepted by Insurance Companies is "spa618". It's a chinese massage parlour and a buddy of mine that tried it out told me that they processed his receipt directly to his company insurance plan with Manulife through the computer for reimbursement. So it does sound legit and provides the standard rub n tub service.

Hope this helps. Unfortunately, I do not generally find chinese women attractive!
