Cannot use voice/chat in halo reach?

Ok..the console, game, controller and headset are bout a month old. All of a sudden I can't talk during a game, in the lobby, friends or anything. There isn't even any voice icons appear next to any ones name in the matchmaking lobby (so outta 16 people..nobody has a mic)? I've done/gone through every preference and setting there is on the controller and system. I recently just got a new headset thinking that was the problem and it still doesn't work, so now I have two headsets that can't be used. I also switched from wireless to wired ethernet connection..still nothing. I just don't know where the problem is. I checked all the mutes and privacy settings, Please Help..
I have tried another controller (one wireless other wired). But I'm not seeing any voice icons next to anyones names either..nothing appears next to anybody's name
Also I sent a voice message to a friend and played it back and I heard it just fine so I know it isn't the headset.

you could also try using a different controller, maybe something is wrong on the controller end.
